Frequently Asked Questions (FAQ) about rTMS

Clear answers to the most common questions about rTMS/iTBS treatment: safety, pain, duration, and when results may appear.

The most frequent questions about rTMS/iTBS in clinical practice. Read below what our patients usually want to know before their first session at Smart CNS Center.

Does rTMS hurt?

The treatment is not painful. During delivery of the magnetic pulses, you will feel a light tapping sensation at the point of contact with the scalp and hear the sound of the magnet, similar to an MRI scanner. During the first 2–3 sessions, there may be mild skin discomfort, which usually disappears as the body becomes accustomed to the sensation. In rare cases, a mild headache may occur and usually resolves quickly with common analgesics.

Is rTMS safe?

Yes. rTMS is approved by major regulatory bodies for specific indications and is used internationally. It is a safe method when applied after proper medical assessment and according to established safety protocols. The main but extremely rare side effect is the risk of seizure, which is why a detailed medical history is taken before treatment starts. It does not produce the systemic side effects typically associated with antidepressants.

Does it require anaesthesia or hospitalization?

No. The patient sits in a comfortable chair and remains fully awake throughout the treatment. No form of anaesthesia, sedation, or preparation is required, including fasting. Once the session is completed, you can leave the clinic immediately, drive, and continue your normal day without any effect on mental clarity.

How long does each session last?

The duration depends entirely on the treatment protocol selected. With the most modern protocols, such as iTBS, the active treatment lasts only 3 to 5 minutes. With classic high-frequency rTMS protocols, the session may last from 19 to almost 40 minutes.

When will I start seeing results?

Although every person responds differently, clinical experience shows that most patients begin to notice improvement in symptoms, such as increased energy, improved mood, and reduced anxiety, after the 10th to 15th session. It is completely normal not to notice major changes during the first week.

Can I continue my medication or work?

Yes. rTMS can be safely combined with the medication you are already taking — in fact, in most cases it starts as an adjunctive treatment alongside existing therapy. It also does not require taking time off work. You can have the session during a break and return immediately afterward.
